Ulthera Before & After

Ulthera uses ultrasound technology to visualize and apply targeted ultrasound energy to enhance the underlying support structure of the skin. Dr. Rokhsar was the 1st in the tri-state area to provide Ulthera, and has  performed more Ultherapy treatments than any other physician in New York, New Jersey and Connecticut. Ulthera DeepSEE system combines two important functionalities of ultrasound waves that make results successful: the ability to both see and treat underlying tissue.  The Ulthera device lays down precise lines of microscopic dots beneath the skin.  Ultrasound energy has a thermal coagulated effect on this support structure causing it to lift and tighten the skin.  Because the body’s natural collagen support structure breaks down overtime and Ultherapy has the additional benefit of stimulating the production of new collagen.

Unlike radio waves or laser treatments, ultrasound has the added benefit of allowing the doctor to see the area he is treating while he is performing the procedure and can adjust the treatment for individuals according to their skin’s needs. It can be used to tighten sagging skin on the face, like jowls and eyebrows, as well as the skin, and other parts of the body. Presently there is a protocol in place for treatment of the forehead (eyebrow lift), face, and neck (non-surgical neck lift), but research is being done to assess the effectiveness of Ulthera on other parts of the body.

Dr. Rokhsar has been using the patented Ulthera device since 2009 to perform non-invasive brow and facelifts, a technique that has now been FDA approved to treat sagging neck skin. The device utilizes ultrasound technology to administer waves of heat into the deepest layers of the skin. The sound waves then target the tissue layers that could only previously be treated as effectively with surgery. The precise, micro-focused heating and vibration of the dermis and deep fibromuscular supporting layers of the skin, stimulates collagen production. Collagen is what gives the supporting layers of the skin its tight structure.

One of Ulthera’s most noticeable uses is its treatment to a problem area often complained about. As the skin ages, it loses its structure and without a strong support system the skin will sag, wrinkle and noticeably affect places like the neck. Unfortunately, when skin ages it loses its collagen, a skin tightening protein that creates a smooth and even surface. The aging process puts the skin through many changes. As cells divide, the dermis, the deepest layer of the skin thins, losing its elastic properties. A once taut and youthful face becomes more susceptible to gravity’s pull when it ages, causing sagging and wrinkles. Instead of springing back when pulled, the delicate skin below the chin remains drooping down the neck. Along with muscle and tissue composition changes, an inevitable turkey neck look develops.

Through the skin, the waves damage microscopic sections of skin at a time. The body responds by naturally creating newer, tighter tissue structures for the skin. Collagen production begins just 30 to 60 minutes after the procedure and continues producing collagen up to two to three months thereafter. Surgical neck lifts and liposuction have for years been the gold standard for cosmetic surgeons to tighten up loose and sagging skin, also known as turkey neck. The procedure can also reduce double chins that are caused by an excess of fat, or in cases where a significant amount of fat is lost and loose skin remains, skin can be tightened and reduced. The procedure requires anesthesia and begins with incisions under the chin in order to separate the skin from the fat and muscle layers. Excess skin or fat hanging down the neck is then tightened and removed. The surgery can last for hours depending on the patient’s cosmetic goals. Sutures are placed in the incision areas that enable the healing process to begin, which can last for weeks.

Dr. Rokhsar uses the most advanced and innovative protocols for the use of Thermage with superior results. Dr. Rokhsar was involved in several research projects investigating the effectiveness of Thermage for skin tightening following liposuction of the abdomen and the breasts.

It has been clinically proven to tighten and gently lift skin to smooth out wrinkles and renew facial contours. The incision-free procedure is fast and easy, and requires no downtime from normal activities. Unlike lasers, the procedure can be performed on patients of all skin types. Immediately following a Thermage procedure, your skin may feel tighter, smoother and more youthful. Over time, you will notice additional tightening. Additional time may be required for skin preparation prior to the procedure.

What is the Ulthera procedure like?

The procedure takes about 30 to 60 minutes depending on the area being treated. Some patients report a hot, prickling sensation, which for many patients can be managed with oral medication given before the procedure.

How soon will I see results?

Using the sound waves, Ultherapy uniquely provides the physicians with an image that is reflected back onto a screen. The sounds waves form a picture of what is inside the body that allows Dr. Rokhsar to actually see as he performs the procedure. The tiny deposits of ultrasound energy stimulate collagen growth below the dermis, which ultimately replaces and strengthens the older collagen. Collagen is the key protein in the skin structure. The more collagen production is encouraged, the more youthful the skin appears to be. The targeted fat is then naturally processed through the body, which provides progressive results that can be seen within eight to 12 weeks. Some patients see lifting results immediately after the procedure, and full results will be seen three months following treatment.

Is there any down time?

Because the Ulthera’s technology treats deep within the skin, leaving the top layer unharmed, there is no down time following the procedure.  It is normal to be feel small knots beneath the skin.  Additional side effects include tingling, tenderness, redness, and flushing.  These side effects are generally mild and temporary. Some patients experience some swelling post treatment, but otherwise patients can return to their daily routine immediately after treatment. The connective tissue begins to instantly tighten, showing visible results within 30 days and taking up to six months to fully respond to the treatment.
