CoolSculpting by Zeltiq is one of a few new options for non-surgically contouring the body.  Dr. Rokhsar was the 1st in the tri-state area to provide CoolSculpting, and has  performed more  treatments than any other physician in New York, New Jersey and Connecticut.With no downtime, it is quickly becoming a sought-after alternative to traditional liposuction procedures. Liposuction has been the gold standard of fat removal for years. But because it is an invasive procedure, a certain amount of downtime must be reserved after surgery. Liposuction is a surgical technique used to remove excess fat from deposits between skin and muscle. After general anesthesia is administered, small incisions are made across the area and a small tube is inserted. A suction pump is connected and gently removes the fat from the incisions, making the total operation lasting between 30 to 60 minutes depending on the patient’s cosmetic goals. Unlike other non-surgical fat removal procedures, which utilize laser technology, CoolSculpting by Zeltiq uses a freezing technology, otherwise known as cripolypolysis. This innovative approach reduces subcutaneous fat without injury to adjacent tissues. Because adipose cells react at different temperatures, CoolSculpting by Zeltiq utilizes a cup with mild vacuum pressure to draw tissue into it. The device then becomes precisely cold enough to target the surrounding fat cells, crystallizing the lipids and prompting cells to undergo a process called “apoptosis,” wherein they shrink and die. This triggers the body’s natural lymphatic debris-collection response, removing the previously damaged fat cells gradually over several months while preserving surrounding healthy tissue and promoting a dermal fibrotic response.

A procedure developed at Wellman Laser Laboratories at Harvard Medical School is based on research that has shown fat cells are more vulnerable to cold temperature than any other surrounding tissue. Extreme cold temperatures are emitted through cooling plates that suction the targeted area for an hour, during which time fat cells are turned from a soft to solid form. Literally freezing fat cells through prolonged controlled cooling, allows the body to naturally metabolize frozen fat out of the body. Dr. Rox Anderson, who developed the concept of CoolSculpting, spoke about non-invasive fat reduction through freezing fat cells. At an international laser conference held in Moscow, Russia in October 2012, attended by hundreds of dermatologists, Dr. Cameron Rokhsar, Assistant Professor of Dermatology at Mount Sinai Hospital in New York, and Dr. R. Rox Anderson, director of Wellman Laser Laboratories at Harvard Medical School, spoke about the status of non-invasive fat reduction procedures currently available.

CoolSculpting and LipoSonix are two reliable non-surgical treatments that are virtually painless and require minimal downtime. While the CoolSculpting device uses extreme cold temperatures to freeze fat cells, which then naturally metabolize out of the body. LipoSonix uses a similar non-invasive approach, but instead it uses high-intensity ultrasound technology to target and heat fat around the abdomen and love handles, which will then naturally metabolize out of the body.

LipoSonix reduces fat volume by focusing high-intensity ultrasound energy on the problem-area, causing no harm to the skin or surrounding tissue and organs, while disrupting fat cells. Once these have been broken down, one of the body’s debris-collection systems, macrophages, engulf the damaged adipose cells. As the fat is disposed of, patients begin to notice a marked decrease in local fat volume.The LipoSonix procedure takes around 30 to 60 minutes for the entire abdomen and is relatively painless and relaxing. Occasionally patients feel cold, heat, tingling, or slight discomfort.

Another fat reduction treatment, which is minimally invasive, can provide patients with more drastic results, is laser liposuction. The widely-known liposuction vacuums fat out of pockets around the body, such as the thighs, hips, abdomen and flanks. Laser liposuction uses a similar approach except a laser is used to break down the fat prior to the vacuuming step in order to make it easier to extract, while stimulating the production of collagen and elastin, which will result in firmer, tighter and smoother skin with less bruising.

What areas can CoolSculpting by Zeltiq treat?
CoolSculpting by Zeltiq works anywhere that patients find fat is difficult to target, most commonly the arms, above the knees, abdomen, waist, flanks, thighs, buttocks, neck, back, and “bra line” fat. Some studies have found a 20 to 80 percent fat reduction of thee areas in just one treatment.

Is CoolSculpting by Zeltiq safe?
Most importantly, CoolSculpting by Zeltiq has been tested rigorously and proven safe. Numerous studies show that the cripolypolysis does not affect serum lipid levels (cholesterol and triglycerides) or liver functioning at any point during or after treatment. Additionally, extensive analysis of peripheral nerve functioning, vasculature, and skin tissue following therapy found no long-term change either subjectively or microscopically.

Who is a good candidate?

What is the procedure like? Is there down time?
Most patients find the procedure pain-free and relaxing. Patients are free to read, listen to music, talk, relax, or watch TV. In fact, a large-scale study found that nearly 95 percent of patients found the process to have little to no discomfort whatsoever. After the application of a thin gel pad on the treatment area, the vacuum system is attached and draws the tissue between two cooling panels. A slight feeling of cold may cause discomfort, but is generally well tolerated, temporary, and negligible. Treatment usually takes only about an hour for one area, and there is absolutely no recovery period. Patients can immediately return to work, exercise, and daily activity, though the skin may be red for up to a few hours, and some patients experience tingling, bruising, or tenderness shortly afterward.

How soon will I see results?
Measurable and noticeable CoolSculpting by Zeltiq results appear in 2 to 4 months, and last around 6 months, with just one session.
